AzTEA
and the Qwest
Foundation for Education have teamed up to reward Arizona
PreK-12 educators who demonstrate an innovative use of technology.

The
Qwest Foundation has allocated to the AzTEA (Arizona Technology
in Education Alliance, Inc) $100,000 through a Qwest Foundation
for Education Grant to be awarded to Arizona PreK-12 educators
who demonstrate an innovative use of technology with students.
The funds will be disbursed to the local school district,
charter or private school. The funds may be used to purchase
the following for use in the classroom: supplies and materials,
technology, or professional development activities.
Qwest
contributes to education in other ways as well. Please take

students.

All proposals will be read and judged by a statewide committee
made up of representatives of PreK-12 educators and business
leaders. This panel will determine those applications that
best meet the funding criteria using the rubric included in
the application package.

How
to Submit Application Electronically |
 |
AzTEA
has created an electronic version of the Qwest Grant application
using the Adobe Acrobat Reader (click
here to download the current version of reader). To complete
this application, download
the PDF and complete the fields included in the document.
At the top of the PDF you will see a button to "Email
your application."
When
you have completed the application, click
this button and Acrobat Reader will open your email client,
fill in the To field, and automatically attach your application.
Simply send the email and your application will be submitted.

IMPORTANT:
AzTEA requires certain assurances by requiring signatures
from you, your principal, and your superintendent. The form
has been set up to accept digital signatures (Click on the
"Sign Here" arrow in the sginature box). However,
we realize that not everyone will be comfortable with this
method so we have also added a "Print Application"
button. After submitting your application electronically,
you may print your application for your records and copy the
first page or you may print the first page only using the
Print option in Reader and obtain the proper signatures. You
can mail the signature page
only to AzTEA at 1753 E. Broadway, Suite 101,
PMB 292 Tempe, AZ 85282.
Your
application is not considered completed until we have received
the signature page.
